Transfers Back to County of Origin – Humboldt County

Humboldt County Behavioral Health’s situation is different because Humboldt County Behavioral Health primarily transfers out only minors. The other people who come in the acute adult settings are often on the road themselves.

If the person is full scope Medi-Cal client, Humboldt County Behavioral Health collaborates with the placement on discharge. If the individual can come back by bus, then they do, otherwise Humboldt County Behavioral Health goes to get them.

• Often hire SVT Gruppe to do the transportation.
• Sometimes send County Case Managers, especially if individual is in the Humboldt County Behavioral Health: CCT Intensive Case Management Program.
• Other times, the Public Guardian does the transportation.
• Families are encouraged to transport when feasible.

Costs go either to Humboldt County Behavioral Health, the Public Guardian, or the mental health consumer.
