POLICIES AND PROCEDURES

Subject: Manpower/Training

EMT-P Expanded Scope of Practice Training

 

I. Authority and Reference (Incorporated herein by reference)

A. Division 2.5 of the Health and Safety Code.

B. Title 22, California Code of Regulations, Chapter 4 of Division 9.

C. North Coast Emergency Medical Services Policies and Procedures.

II. Purpose

To provide a method by which eligible individuals may obtain training, written testing, and skill demonstration necessary in any, or all, of the North Coast EMS approved EMT-P Optional Scope of Practice prior to local accreditation. These are:

A. Medications

1. Adenosine.

2. Glucagon.

3. KCl < 40 meq/l.

4. Magnesium sulfate.

5. Neosynephrine topical application during nasotracheal intubation.

6. Procainamide.

B. Procedures

1. Carotid sinus massage.

2. Intraosseous infusion.

3. Pediatric laryngoscopy and endotracheal intubation and/or removal of foreign body with Magill forceps.

4. Pediatric endotracheal suctioning.

5. Pediatric endotracheal extubation.

6. Nasotracheal intubation.

7. Needle cricothyrotomy.

8. Determination of blood sugar by finger stick.

III. Procedure

Individuals will be required to obtain training and testing in all items of the North Coast EMS approved Optional Scope of Practice. The individual shall complete the following self-paced method described below:

III. Procedure Continued

A. Procedure related items

1. Obtain training packages (appropriate policies) from North Coast EMS.

2. Arrange to obtain required training and skills testing.

3. Training may be offered by the individual's assigned Base Hospital or North Coast EMS approved training institution (such as Redwood Coast Emergency Training Center or authorized EMT-P training program.

4. Arrange with North Coast EMS for required written testing on designated test days (tests may be administered by the Prehospital Care Nurse Coordinator (PCNC).

B. Medication related items

1. Training packages(appropriate policies) can be obtained at North Coast EMS for required items.

2. Arrange with North Coast EMS for required written testing on designated test days.

IV. EMT-P Optional Scope of Practice Requirements

A. Documentation required of successful completion of training.

B. Prior to local accreditation as an EMT-P in the North Coast EMS region, the applicant must be trained in the use of all medications and procedures as outlined above (Also, see Accreditation Policy).

C. Prior to local accreditation as an EMT-P in the North Coast EMS region, the applicant must take a written test, which covers the expanded scope of practice and the appropriate skills tests.

D. Upon North Coast EMS receipt of documentation and accreditation, individual will be authorized to utilize optional scope of practice. Notification of local accreditation will be made by the North Coast EMS Medical Director to the EMT-P, and his/her provider and Base Hospital.
